If you would like to telephone verify all orders, then you could place the form on the same page where you ask for the credit card details. If you would like to only telephone verify orders that exceed a minFraud fraud score threshold, then you could place the form on a separate page after the payment information is submitted and you have run a query against our minFraud service.
