Consumers and their authorized agents can use the following form to request the deletion of their personal data. You may use this form if you have been referred here to verify a request you have previously submitted to MaxMind.

In order to process and validate your request, please provide the following pieces of information. The pieces of information you provide below will only be used for purposes relating to processing your deletion request including verifying the identity of the data subject and complying with MaxMind’s record retention obligations.
